Application Procedure

The deadline for the receipt of all application materials (application, references and official transcripts) is February 1st.

This checklist can be used to ensure that you meet the Augusta Health School of Clinical Laboratory Science admission requirements or to help you when submitting your application materials.

Preferred method of submission: Type your answers directly on the form and submit the form as an email attachment to: ecoggins@augustahealth.com.

3. References

Download the Reference Form

Print three copies and provide one form to each selected reference. Each reference may mail the forms directly to the address above or they may return the form to the applicant for submission provided the reference letter is submitted in a sealed envelope with the reference's signature across the seal.

4. Request official transcripts from all colleges and/or universities attended

These may be submitted directly from the college/university to the address above or may be submitted by the applicant provided the transcript is a sealed official copy.

5. Scholastic Achievement Scholarship

Qualified applicants may submit the completed form as an email attachment or via postal mail to the address above. Students who meet selection criteria may be eligible for an initial $800 scholarship award upon acceptance into the program. Additional scholarship and work grant monies may also be available following posted application deadlines.

6. Financial Need Scholarship, Work Grant, and Tuition Assistance

Applications will be provided to students after acceptance into the program.
