Community Health Needs Assessment
Community health is multi-faceted and it will take the concerted and coordinated efforts of all organizations in our community to truly make a difference in improving community health. Augusta Health, as part of its mission to improve the health and well-being of its community, partners and collaborates with other key organizations to improve the lives of people in our community.
In 2012, Augusta Health began a Community Health Needs Assessment to identify the major health needs of our community and to focus the efforts and resources of the hospital to the best possible use. Stratasan, a firm that specializes in facilitating community Health Needs Assessment plans, was hired and completed the data assessment phase.
A Steering Committee comprised of key leaders in the community was established to oversee the process and provide input. Stratasan compiled a data assessment that from several sources of secondary data to provide an overview of the health of the community. Stratasan then conducted primary research through a community telephone poll that included a broad spectrum of the community citizens and a physician survey for opinions on health issues in the local community. Two focus group meetings were held to obtain input from individuals who represented major sectors of the community. Finally, two major Summit Meetings were held, allowing all community organizations and agencies and the public to participate in defining our health needs.
The preliminary data from Stratasan is posted below. The comprehensive Community Health Needs plan that is developed by Augusta Health and Stratasan will be posted when completed and approved. That plan will include measurable objectives, initiatives, implementation strategies and measurements. This plan is expected to be completed by the end of May 2013. It will also be distributed at Community Forum meetings.
