Who is Eligible?

You are eligible if a physician provides written orders and certifies the need for Home Health Care.

You must be homebound. This means leaving home requires a considerable and taxing effort, in most cases, requiring the aid of a cane, a wheelchair, a walker, special transportation and/or the assistance of another person.

You must require intermittent skilled care such as skilled nursing, physical, occupational, or speech therapy.
