Care Partners are the organizations that collaborate together when caring for our community. Hospitals, walk-in clinics, primary care, and specialty care offices are all involved.
ICD-10 is an important update to the way we code patients' medical conditions and procedures. It went into effect on October 1, 2015. The ICD-9 system is outdated and has limited room for growth to keep up with current medical practices. With ICD-10 we will be able to standardize data across a patient's care continuum, leading to more accurate diagnosis, improved treatment plans, and ultimately a healthier population.
