Referral & Insurance

A referral from your physician is required for outpatient nutrition services. Referrals should be mailed or faxed to the Diabetes and Nutrition Self-Management Education Program. Please include any supporting documentation, such as lab results and progress notes.

Insurance pre-authorizations should be obtained prior to scheduling the appointment. We advise contacting your insurance company to ensure you are covered for any service being provided.
