Augusta Health is focused on enhancing the health of all individuals and families and strengthening the community we call home. Community Outreach leads initiatives such health screenings for those at-risk for multiple chronic diseases, programs designed for those with barriers to accessing healthcare services and services designed for those who are food insecure.

Supporting graduating high school seniors and adults seeking higher education
Based on findings from the latest Community Health Needs Assessment (CHNA), we create focused community initiatives that address the fourteen areas of greatest need identified in the assessment. These initiatives help ensure that we continue to promote the health and well-being of our community while serving our neighbors who need us most.
