To ensure the safety and well-being of our patients, their families, and staff during the coronavirus (COVID-19) visitation restrictions, the Medical Records office is temporarily closed to walk-in customers. If you need copies of medical records, please follow the steps listed below. Completed requests can also be emailed to medicalrecords [at] augustahealth.com
Medical information is the lifeblood of the healthcare delivery system. The medical record contains all of the medical information that describes all aspects of patient care and serves as a communication link among caregivers. Documentation in the medical record also serves to protect the legal interests of the patient, healthcare provider, and healthcare facility. Medical records are important to the financial well being of the facility as they substantiate reimbursement claims. Other uses of medical records include provision of data for medical research, education of health care providers, public health studies, and quality review.
It is the responsibility of Health Information Management to assist in compiling complete and accurate medical information as well as safeguard this information in a complex and ever changing healthcare delivery system.
Requesting Copies of Medical Records
To request copies of medical records, please complete the Patient Authorization Release
and return it to our office in person or by fax.
