- Referral for Outpatient Physical Therapy listing all pertinent diagnoses
- Most recent office visit notes, which should include documentation of the need for a mobility/seating evaluation and any medication lists.
- Located on the ground floor of Augusta Health Main Hospital in the Inpatient Therapy Department. Valet parking is available at the main entrance of the hospital.
