Home Health gives patients the opportunity to remain at home and receive treatment under the supervision of their physician. Using a team approach, our goal is to help you live independently at home while you recover from an illness, injury or surgery or manage a chronic medical condition. We provide continuity of care as you transition from hospital to home.

Who is eligible?
- You are eligible if a physician provides written orders and certifies the need for Home Health Care.
- You must be homebound. This means leaving home requires a considerable and taxing effort, in most cases, requiring the aid of a cane, a wheelchair, a walker, special transportation and/or the assistance of another person.
- You must require intermittent skilled care such as skilled nursing, physical, occupational, or speech therapy.
